Transcript Request Form

For transcript requests of current school year (2010-11) and last year’s students (2009-10), please contact the Osseo Area School of last attendance.

Former students (2008-09 school year and prior) of the Osseo Area Schools may request school transcripts for employment purposes, post-secondary education, identification purposes, etc. Complete the information below, press send, and your request will be forwarded to Student Records for processing. Requests may take 5 to 7 business days to process.

Official copy of transcript: required by most colleges, universities, employers, etc. This is an official copy of the student’s transcript with an embossed district seal and mailed directly to college, employer, etc.

Unofficial copy of transcript: generally for individual use; not accepted by most post-secondary institutions or employers.
